Ce este transferul?What is transference?

In the classical Freudian tradition it was seen as distortion - a projection of the past, to be corrected. Quatman, in a relational-intersubjective vein, reframes it: not an error, but a structured repetition of internal relational models, which becomes the central clinical material. We no longer “correct” transference; we use it.

And it asks for listening “in stereo”: attentive at once to the patient and to one's own inner reactions, because shame, hard to symbolise, is often communicated not through words but through its affective impact on the therapist.

A fine technical detail: the therapist puts into words the experience of the patient with the therapist, not the reverse. You cannot change someone who feels unworthy of love by telling them you love them; what transforms is that the patient themselves comes to articulate and examine that experience.

Quatman's guide is disarmingly simple: if you attend to what it is like to be with this patient now, and what it is like to be the patient in relationship with you now, you are tracking exactly what matters.

And the therapist's countertransference - the diffuse discomfort, the causeless irritation, the defensive urge to soothe - becomes one of the few signals through which hidden shame can be detected before the patient can name it.

The patient speaks freely about a situation at work. The therapist makes a seemingly neutral observation, in a calm tone: “I notice that in this situation you chose to say nothing.” The next second, the patient falls silent, avoids eye contact, the rhythm changes; the answers become short, monosyllabic. At the level of transference, the observation was felt as a criticism - shame has been activated but is not verbalised; the signal is in the relationship, not in the speech. At the level of countertransference, the therapist feels a diffuse discomfort and the urge to “repair”. These reactions are clinical information.

The finer, relationally oriented: “I wonder whether what I said was felt in a difficult way by you.” It does not presume but opens; it does not accuse but acknowledges that something happened between us worth attending to. The same scene, two interventions - and radically different outcomes. The whole architecture of the two texts fits into this minute: shame structured the perception (Hultberg), showed itself in transference and was detectable in countertransference (Quatman), and the good intervention was the one that kept the relationship alive without intensifying the exposure.

Not every emotion in the session has clinical relevance. That is why countertransference is treated as a working hypothesis, not an immediate truth, and validated by three criteria: consistency over time (an isolated emotion is not information; a persistent, ego-dystonic one is a signal), correlation with the patient's material, and reflection in supervision. The subtle forms of countertransferential acting-out - the wish to “rescue”, the avoidance of difficult themes, emotional withdrawal - call for the same vigilance.
